Hello bakulaparikh,

Upon checking your account I found that it's suspended by our anti-phishing system. As your form "Signup Form" has fields like "Email & Pass" which makes it suspected of being used for phishing.

Authentication form can't be used in JotForm and creating such forms are against the Terms of Use. I have deleted the password field and activated your account again. I would suggest you to please test your form again and get back to us if you do not receive submission emails.

Kindly refrain from creating form with email/password, login/password or credit card details.
