Submission: Modifying style of PDF submission

    Is there a way to make an online form LOOK like the PDF Form i attached? 

    So far I've made it with your form designer and it looks good and all but when the form is filled out and emailed, it isn't in the same format.

    It looks very different in the "received email" side of things.

    Hi there,

    The PDF submission of a form which becomes an attachment to the email autoresponder/notification when someone submits your form does not automatically look like your form. By default, the PDF submission's format will be generic. You will need to customize it by going the the submissions page of your form - https://www.jotform.com/submissions/70579035437158, selecting PDF > Customize PDF.

    In that screen, you will be able to modify the PDF submission of your form.
