I cannot see the survey I created due to securty alert.

    Your form is shown properly on my end.

    However it seems that this issue is related with Turk Telekom and form.jotformeu.com was blocked in Turkey. Because of this reason, you encounter this problem.

    To resolve your issue try to view your form without "eu"?

    Instead of this link:

    https://form.jotformeu.com/43495826534363  

    Please try this with link:

    https://form.jotform.com/43495826534363

    You can also provide us your external IP and DNS details and we will raise this issue to our developers.
