I am receiving 'Form not found This form is disabled.' error

PROBLEM WITH THE FORM SAID

I see that the account is auto-suspended by our anti-phishing system which is strict to scan each form to calculate the probability of phishing activities. I notice that your JotForm is collecting Credit card information using plain text fields. It is not allowed to collect credit card information on plain text fields using JotForm. If you need to receive payments, it is required to use one of the payment methods available in the form builder. These payment methods comply with the PCI/DSS security standards to process the payments securely.

I have removed the guilty fields from the form and activated your account. Please refrain from adding such fields to your forms to avoid any further suspensions on your account.
