Report of JotForm user violating TOS

Hello, This jotform user is collecting credit card information.

My understanding is that this collection of this sensitive information is not allowed by jotform TOS:

Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials.

Can you please take action to help this jotform user better comply with the jotform terms of service.

Thank you for reporting this to us. I've checked that form and the other forms of the user account cyouonop, that indeed it violates our terms of use. I have suspended the account. If you find similar forms, please do let us know. We do have an anti-Phishing system but not all of the forms can be scanned properly as we have hundreds of thousands of form. But rest assure that we are constantly improving the system for this kind of forms and violations.
