Keep getting "Form is Disabled" alert

    I keep getting the "Form is Disabled" alert when attempting to access forms that are enabled.

    Your account was auto-suspended by our Anti-Phishing Tool and made your forms disabled. I checked one of your forms and I noticed that the form (http://www.jotform.us/form/70540699588169) has fields that asking for Credit Card details to the users. Please note that asking such details to the users is against our Terms of Use.

    If you want to ask payments from the user, you can use our Payment Processing Gateways instead. Here's the link: Payment Processing Apps.

    I removed now the guilty fields and re-activated your account. Please refrain from doing this so that you will not be suspended again.
