My account is suspended and I have no way to reactivate it

    I can't access my account. One of our forms stopped working and now I try to log in and it says suspended.

     

    What do I do?

     

    The account username is advisorwebsites and the account email is support@advisorwebsites.com

     

    Please help thanks!

    I have inspected your account and found that your form ID 70474679954271 named "QFS" is collecting logins and passwords, in violation of our terms of use.
