We've used Jotform for a while now for our church. We just discovered that our forms are NOT sending emails as they should! We've missed over 50 Form submissions between Oct 2016 and March 2017! Only 1 or 2 made it through!
We also discoverd, through the new JotForm 4.0, that our form was listed as "Disabled on Date: 1899". How did this happen? (We didn't do it) I re-Enabled the form and performed tests, but we are still not receiving emails with the Form Submissions.

I cannot find any evidence that the submissions were not delivered, in fact our mail logs show a green flag indicating that all submissions were successfully sent.
Can you please double check in your spam folders for the emails with domain @uugsb.org?
On regards to the form "Disabled on Date: 1899" , your account does not list such form name. Can you please give us the link?
