Ddslick dropdown widget is not responsive

The ddslick dropdown widget on page 9 of my form, or Device Information is offset on my mobile version.

The form you provided here above was deleted. However, I checked mentioned widget on mobile view and I can say that there is no responsiveness issue. Please also note that Jotform Version 4 has a default mobile responsive feature and you do not need to add mobile responsive widget. Here is the result of how widget is seen on different devices. 

You might have added some other css which cause a responsive issue? Could you please provide us some details or provide an active form url which has this widget and this issue?
