Error. Form disabled.

Have been trying to create a form for my sales people where they can enter orders, and send their location at the same time, but suddenly it says form not found on the view log.

I have tried everything from the settings, what is wrong.

The system found this form suspicious of phishing activity and automatically suspended it, but after checking further I have unsuspended it.

Note: There is no need to post multiple threads for the same issue, all of the threads are answered in the same order they came in.
