Salesforce Integration issues

I am trying to remove the Salesforce integration and add it back but the remove button is not working. Also the SF integration is not completing. 

Please make sure you were logged in to your account as well when loading your form in the Form Builder.

Can you please tell us the URL of the form that have the issue. We will look into it more once we determined which form.

I can only assume it was correct that it was the form I see in the activity logs. I did reviewed the form and I can see it was already using Salesforce integration and the option to remove integration is available as well.
