How to add multiple participants in form?

Hi

We are looking at JotForm, and have made some test-forms. We are considering to use JotForm for registration to a dental seminar.

A dental office may register 1 to 15 participants. First they fill inn the information about the office and then they add the participants (name, profession and which course to participate in). Is it possible to add a plus-sign or a button to add fill inn the participants?

The best way would be to use Configurable list widget https://widgets.jotform.com/widget/configurable_list with that widget you can set the fields you need for each row and add as many as you want by using the plus sign. Something like this:
