Its' a caching issue when using http://form.jotform version of the form. Replacing it with http://www.jotform should fix it.

So, replace the embed code in your website with this code:

    <script type="text/javascript" src="//www.jotform.com/jsform/20195142993"></script>

    Let me know.

The forms will update eventually (delayed), so you don't need to to do this on every form. You can apply the immediate fix to some forms.

We are having a slight problem with the cached forms on our CDN (content distribution network) and the problem has been already forwarded to our Development Team.

    Sorry for the inconvenience.
