Why my forms are disabled?

    I've created 4 forms on my account. While I was editing one, when I refreshed the page it then says my form is not found and was disabled. I tried opening all my forms and they were all disabled. Can you please help me with this?

    Your account was auto-suspended because you had Username field in the form and it's against our Terms of Use to collect such information:

    I removed that field from your forms and please avoid putting it back or your account will be suspended again.

    Also, since you reached your Payment submissions limit your account is now Overlimit until the next first of the Month or you Upgrade.
