Why are my forms disabled?

Hi, Please help! Now two of my forms are disabled and I have already disseminated the links throughout our digital footprint!

The problem is that you are requesting login credentials from websites, facebook and other social media sites in the forms. This is an infringement of our Terms, so please refrain from doing that.
