Are the health related forms HIPPA compliant, or can you suggest how to use the form so that the information provided is HIPPA compliant?

I'm trying to determine that if I use the forms on my website and submit the information to my email address,  will the information provided by the client be HIPPA compliant?  Thank YOU!

We do not enforce compliance so it would be up to you to use the form in a compliant manner.
