How to keep track of which website a generic form was submitted from?

I'm hoping to make a donation form for use on each of our community partner's websites, and all forms will be submitted back to my department. I need to know which form is submitted from which webpage. Is there a way to discretely label each form? One for the food bank, one for united way, and so on... so when the form is submitted I can keep track of where the form and donation came from.

    Thoughts?

You can label each of your forms and make the label hidden, so it is visible only to you, for example:

Or even better, edit your Notifier:

And add a line that you can use for easier identification:

Notifiers are something that only you can see. So it might be a better fit.

Please let us know how it worked for you.

    Thank you!
