Why are all my forms showing disabled?

    My forms have been disabled and I am not sure what has happened to make this so.

    They were working a few weeks ago fine but now just show a form not found message.

    Dear Luke,

    The account is auto-suspended because your form CLIENT UPLOAD you are requesting password in plain text field.

    Please be advised that to collect this type of data in plain text fields will violate our Terms of use, as this is only allowed through encryption along database administration; and our forms do not provide this type of method.

    We have removed the guilty fields on this form and reactivate the account. You must refrain from doing a new form like that to prevent a potential suspension of your account.
