Original form with integration to new template has major issues with layout and look

    I'm operating a business that needs to have up and running reservation form.  I thought we could edit and make changes but it made the entire form into something we can't even control.  The look of the layout looks fine on the edit page but once in preview and online, it's a mess.  

    I didin't have this issue before but there's this really big border around the form and when I use embedded iframe, the whole border takes up too much space.  Not only that, once in iPhone to view form on our webpage, it's a complete disaster.  The forms do not look as if it is designed and trying to start from the beginning to create a new form does not show on iPhone.  

    Please help as our business is operated on this form.  Thank you for your attention.

    I checked your form and it seems like the design is still there, here's a screencast of it:

    When I viewed your website on a mobile device, I see that the website itself is not mobile responsive, so there might indeed have issues viewing it on a smaller screen. Here's how it looks like:

    I also see that the theme you are using is from our old list of themes. You can try going through our Themes Store page and choose a new theme to redesign your form. 

    Let us know if you are still having problems, please do try clearing your browser's cache just to make sure.
