Hi
I would like to create a form where the user enters the relevant info (first name, company name, etc) and upon pushing Submit, the submission results in a letter where the information from the required fields are integrated with text. For Example, the input screen may look like this:
Date
First Name Last Name
Company Address
Dear First Name
As per our discussion on Date, we have agreed to the following terms of engagement:
- Term 1
- Term 2
- Term 3
Your engagement with us commences on Date and must be completed by Date. We look forward to working with you.
Regards
Managers Name
Company

Once the form is submitted, an auto-responder will be sent to the email address provided in the form. You may consider customizing the Auto-responder to the required format. Please refer to the guide below that can help you with adding an auto-responder to the form.
