it was working all of last year and now it stopped. I stripped all of the fields and I am trying with just 3 fields and it is not integrating please advise.

Make sure that the fields are properly mapped as SalesForce is very exact when it comes to mapped fields, so the same fields have to exists both on SalesForce and your form.

The integration works fine, but based on our logs, it seems that the entries with name "David Genaro" simply overwrites the existing one because your integration settings has 'Duplicate Check' set to 'Always Update Existing Entry'.
Can you please check if an existing 'David Genaro' lead is in your SalesForce account?
