This is a most common problem when you're not logged in to JotForm. Please make it sure that you are already logged in to JotForm before creating your forms, and then click 'SAVE" button when you are done with your form. I would also suggest to clear your browsers cache first before logging in to your account, this should really help specially when your old cache files are broken.
With regards to your other concern, instead of using dreamweaver source code, you may try downloading your form's full source code. This would include all JS and CSS files. Follow this guide: How to download a source code of your form
Hope this helps. Thanks for using JotForm!

Thanks for the details. Is downloading the CSS not suitable for your needs? I can see the error is looking for this file: http://max.jotfor.ms/min/g=formCss?3.0.3661. How about copying this CSS code and use it locally?
Let us know if this doesn't help. Thank you.
