A form that I want to use has a consistent "Error loading" issue

Hello,

I am trying to use a Girl Scout cookie form template, but I am unable to use it because it keeps saying there was an error in the page loading. 

When I was finally able to edit it, it said the form was not available for preview or publishing.

I tried different browsers to no avail. Can you assist?

We bug that caused forms to not be editable in V4.  It has been fixed and you can now edit forms once again.  Apologies for the down time.

As for the form being unavailable, it looks like it was manually disabled.  Set the back to enabled and it will be usable again.  To do so, while in the "My Forms" section of your account, select the form, click on "More" then click on "Enable":
