I'm having the same issue as above on any form I create, the Docusign account is authenticated but I always receive the message "Something is wrong please click here to try again"
We have not reached our sending limit on Docusign so not sure what's causing the issue.
Here's an example form:

I have tested the DocuSign widget on my test form and see that it is working fine at my end.
Could you try re-authenticating the DocuSign account on the form and see if that helps? If you are testing with a Free DocuSign account, you are limited to only 3 sends which you may have completed. You may need to check the DocuSign account status. If the issue still persists, please let us know. We will be happy to help.
