Shipping Address has disappeared

    Hi, I created a form which includes a shipping address.

    Upon finalization and publishing - when I preview - the shipping address is not included anywhere on the form.

    Help please!

    Also, does a paid membership include LIVE customer support?  Thank you.

    The shipping address does not appear, because you have setup a condition for it to show only if the answer for the field "Send Gift" is equal to "Yes": https://www.jotform.com/help/316-How-to-Show-or-Hide-Fields-Base-on-User-s-Answer
