10 sites, 1 form - set default site in form for each

    Hi,

    We have 10 sites which are required to submit the same form each month.

     

    I have a location list drop down with the 10 sites.

     

    How can I make this drop down default to a particular location and have it greyed out.

     

    i don't want to create a separate form for each site as I want the data to be consolidated.

     

    also, is there a way I can require a user to enter a password or otherwise validate themselves before submitting the response?

    Unfortunately, we do not have that kind of feature in the form builder as far as I know. However, I think dynamic drop down may still be useful http://widgets.jotform.com/widget/dynamic_dropdowns. Or you can use this widget which disables option after it's selected, http://widgets.jotform.com/widget/asmselect, though you can't make conditions in it that syncs on the other field. It's not possible.

    Furthermore, you can always implement the function you want by applying it manually to your form source code. http://www.jotform.com/help/104-How-to-get-the-Full-Source-Code-of-your-Form. You can take your form source code and inject scripts into it.

    Password field are not allowed as well as the Username field as our Anti Phishing tool will automatically suspend your account due to violation of Jotform's Terms of Use
