What would cause a blank record in a submission spreadsheet?

Attached is a file showing submissions to our Synagogue Security Contact Form. Row 8 is completely blank. What would cause this? 

I have a notification email that is time stamped at 3/22 11:46 a.m. that would correspond to the blank form in row 8 that is time stamped at 11:45 a.m. The transaction email contains responses to all of the fields. (I can forward the email to you if you like - tell me where to send it.)

Unfortunately, the files other than images cannot be attached to the forum post. However, I take a look at the form submissions and see that the submission received at 11:45:40 is displaying as blank. But the email log is still displaying the submission with the data. Could you check if the submission has been edited? Please confirm so that we can forward the issue to our backend team to check. Also, let us know if you notice such issue with any other forms in your account.
