A form has been flagged as phishing - please help

    We have a banking form that we are using to collect banking data from our grant recipients, and I received an e-mail attached below.

    Could someone please contact me so that we can resolve the issue as soon as possible?  We are currently in the process of collecting this data in other forms.

    Here at JotForm, we make and extra effort to educate our customers in regards to our Terms of Use.

    The form "PAS Congress 2017 Fellows Scholarship Claim Form "was found on your account as suspicious of phishing activity by our automated Phishing detector system.





    You agreed upon acceptance of the Terms of Use that the Company ( meaning JotForm) may terminate your JotForm service immediately if a form's purpose is found to be deceptively obtaining sensitive information, such as and not limited to: credit cards, Social Security Numbers or login credentials, or any other sensitive personal information such as banking sensitive coding.


    Please be aware that this is illegal because it's considered a phishing activity, which violates our terms. Card number, expiration date, security codes and card holder name are considered critical information.  The only services allowed to collect  such data are those that are DSS/PCI certified.



    For the matter, it could be the way you set up the above mentioned form was not really for performing any phishing activity, so we would like to inform you we have implemented genuine payment integration tools that will allow you to smoothly go through the process of getting a payment from your customers without infringing any law or putting our company in jeopardy.     We strongly suggest you to removed the guilty fields on this form, as soon as possible, we will follow up on this issue within 24 hours, in case those fields are present, we will be forced to remove them all.

    Unfortunately, will not be possible to call you, we do not provide phone support and the better way to contact us is through our forum. 

    Do note that asking for baking information cannot be stored in your forms and this is why you received that email. 

    There're exceptions to storing banking information in plain text fields and you need to send us a proof that your business is related to one of the following: 

    Lending Institutions. 

    Real State Broker. 

    Mortgage Broker.

    Providing the proof will allow us to get the fields in your form, otherwise they will need to be removed.
