I have heard good things about this service. I am minister and my church is having officer nominations for the upcoming year. We have a ministry interest survey form that members can complete and submit if they have a desire to serve in a ministry. The problems is that this was all hard copy and used a lot of paper and time intensive. Can jotform accomplish the task where I have the members go online or via email link complete the form and the administrator, myself or designee, receive the information for each person. I really need some help. We are trying to streamline the process. I have uploaded the hard copy form for you to look at.

You will receive the information submitted via e-mail. Data is also saved on our server and is exportable to Excel or CSV format.
For the paper form you have attached in this post, you may use checkboxes for the items such as "Elders, Education, Deacons, etc". We also have built in tools for "Full Name", "Phone Number" and "E-mail".
