Why my account is disabled?

I am creating a form for store boys to enter order details and they need to enter a password. I have created this with conditional logic so that if they enter their phone number they can log in. But now I cant preview my form and it get the jotform error this form is disabled please enable my form so that I can progress with my work... This form will be used internally by store boys so no high level security if need I only need to verify if they eneter their correct phone number

Your account was auto-suspended. I checked your form and confirmed that it is not used for phishing activities so I re-activated your account. I'm not sure if you did, but please do not put username and password fields to your form since it is against our Terms of use and it can trigger our anti-phishing system and disable your form.
