My forms are "disabled"

    Hi

     

    I have been editing some forms and added a new one, and they won't show - marked as "disabled"?

     

    Please assist.

    Your account (multicoach) was auto-suspended by our Anti-Phishing Tool. That's why your forms have been disabled. I checked the Forms in your account and I noticed that the Form (http://www.jotform.us/form/70866090993165) has a password field. Please note that asking username and password from the users using the form is against our Terms of Use

    I have now deleted the guilty fields on your form and re-activated your account. Please refrain from doing this, so that your account will not be suspended again.
