Form is disabled

    Hi, I have just set up a form and it says it is disabled when i go to view it.
    Please check what the problem might be, I could not find a place to enable or disable anywhere.

    I have checked and your account is autosuspended by our phishing system. Because of that your form is disabled. 

    I have checked and one of earlier versions of your form had credit card fields and it is against our terms of use to collect credit card info on plain text fields.

    Phishing - You agree that Company may terminate your JotForm Service immediately if a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately.

    Card number, expiration date, security codes and card holder name are considered critical information and cannot be collected on plain text fields. The only services allowed to collect such data are those that are DSS/PCI certified.

    Considering that you have removed those fields and that you are now using JotForm payment tool I have activated your account.
