Whenever we try to go to it we get the form unavailable message.
Then when i went to look at it, it looks like it was somehow changed to not have any fields (which isn't true) and hitting revert doesn't help.
that's the form
and it's not just me having the issue. Please help ASAP.

The form shows as Unavailable because it is set to get disabled when 20 submissions are received:
You may go ahead to remove that option from Settings, Click at Form Status and select Enable.

I did check your form and found that you have already removed the "Submission Limit" of your form. Your form is back online and should work as expected.
