I can get in from my phone but not my iPad, is it likely because I attempted to use a W-9 from the IRS as a form?

It looks like that form triggered our anti-phishing filter to suspend your account. I notice that you have already deleted the form from your account. However, you'll be needed to purge the form from the trash folder to delete the form permanently.
I have now activated your account so that you should be able to login to your account without any issue. Please make sure that you delete the guilty form from your account to avoid any further suspensions.
