How to change the form URL?

I am trying to change the URL for our Digital Agency Form and whenever I go to customize the link it says that it was successful, but then actually links to the original form that was duplicated. How do I fix this?

I've checked the custom link to your form and I can see it redirects properly to the correct form, I can also see the publish wizard provides the correct form's link, this is what I can actually see:

No I want to change the form to /digitalagencyform but when i do that it links to the platinum activation form instead.

I've checked your form and I can see you have not created your custom link, it's still using the direct link. 

Are you generating the custom link as shown on my screenshot above? 

I've checked this and I can see it redirects to the correct form when using the custom link, as I can understand you have cloned this form from another one.
