How can I create a sheet that will summarize findings in a final form based on my choices earlier in the form?

    i'm looking to create a homework plan for clients whereby I go through a physical exam and click off applicable boxes for what I want to be summarized in more detail in a take  home sheet. Example: sit to stand and cookie stretches are clicked and on the last page, a pre-written explanation of these exercises pops up and is  summarized for the client.

    Hi there - I do have a couple more questions, thanks. If I'm having the thank you page be where the summary occurs based on selections - can I create a multiple page thank you page? Using conditional logic, I can currently select one option and have more detailed info pop up on the thank you page. This works if I select one option at a time, but if I select multiple options to pop up on the thank you page.. only one selection shows up. I'm not sure if this is a thank you page size problem or if I'm doing something incorrectly. So, I can click on "ortho/neuro precautions" and my image shows up, but I cannot also click on the home work page at the same time and have it show up as well. The homework does show up if I click only on that choice.

    You cannot redirect the form to two thank you messages/pages after submission. You can only show up one thank you message/page. A workaround would be to show the image or text in the form instead of showing a thank you page.

    For the image, you can use our image field. For the text, you can use our text field. To show/hide the fields, do it by conditional logic.
