My form is not getting submitted

Every time they hit submit it gets re-directed and does not finish.  Please help.  I just did this and it worked but I have had several complaints that it did not work.

I have tested your embedded form and I was not able to replicate mentioned issue. I successfully submitted the form and I got redirected to Thank You page.

Where are users that reported this issue redirected to? Are they receiving some error message when they click on submit button? If so, please provide us a screenshot or that error message or a screenshot of the page where they are redirected to.
