I got this response back. How can I remove the form if I can't access the account? Please reactivate.Answered by Sven
You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials.
In order to get your account re-activated, these fields need to be removed.

We also noticed that you added a signature field to ask for customer agreement, this is not going to make it legal. Why? because the correct way to encrypt credit card information is through payment integration:
You might have their consent but the information will still linger in text fields, which is not allowed.
We have activated your account, cleaned your form. You may continue to develop it.
Please refrain from collecting such information. Try to apply the process mentioned above.
