Can you help me move a form to another account?

    i want to move the form "VB Personal Ad Form" from my jblovits account to my SalineVarsityBlues account, so that i don't have to recreate it.  Can you help to copy this form over.

    The request should be coming from the form owner for security and tracking purposes. Please login to the account owner of the form (jblovits) and make a form transfer request.
