How can I remove the pre-filled text boxes and pre-selection of radio and multiple choice boxes?

I do not want to give the user any pre-filled text boxes or pre-selection radio or multiple choice boxes.  How do I turn these off.

I have checked your JotForm at my end, it seems to be displaying correctly with blank fields. There is no data filled in the form fields. If you have the option Continue Forms Later option enabled on your form and then tried filling the form, the data will be displaying on the form when it is opened in the same browser. You may try opening the form in a separate browser to see the form displaying correctly. You may also clear the browser cache so that the form will be loading fine.
