My Form that I Just Created Is Now "Disabled"

I just about finished up setting up my form and suddenly when I go to preview it I'm getting a message saying:

"Form Not Found

This Form Is Disabled"

I've checked and it's definitely enabled, published, and everything.  The problem seems to have arisen after I modified something in the email notifications section.

I removed the parts that caused your account marked as phishing. Please update your form that it won't contain password fields.
