Why do my forms don't have all the info I put on it. I ran a test form and info is missing from the test application when I check the email..?

    Hello Shawanda,

What is your JotForm account username or the email address you have used in your account profile. I am unable to find any details with your email address slathomebusiness@gmail.com

Alternatively you can also let us know your webpage URL where you have embedded your form and we will take a look. I am unable to find any JotForm embedded in your website. I would also suggest you to please share a screenshot of the issue. The following guide should help you how to upload image in forum post:  http://www.jotform.com/answers/277033

We will wait for your response.

    Thank you!

    Thanks for responding JotForm!
My user name is SMLVS. I have a screenshot on what the form looks like and
how my website looks when I turn the form on...
I did a test and imputed info in the whole form but as you see info is
    missing.
it makes my featured video very small compared to the origianal size. Only
when I turn off the jotforms my featured video goes back to the normal
size. It should fit across the whole top portion of my pages on every
page. Below are some screenshots of what my website page looks like when
the forms are in use and notin use.
    Thanks,
    Shawanda Lamb
This form does not show all of the info.I can see all of the info on my
    other pages...
This website' s video page is small... on my agent deposit page.
The video is very small on my existing csp page.
This is how my video should look on every page.
