Hi,

    Yes you can embed the same form multiple times on the same page.

    We can assist you better if you can at least share to us the URL of the website where the form was embedded. It seems the one you provided http://stormfront.co.uk/smadamij1973/education/mac/ is not working anymore

    Will await your update

    Thanks.

    update on this issue?

    when i use this version its fine, cuz it has two difrent jsform numbers at the end of the url

    but when i try to use the same exactly duplicate jsform number, twice times on the same page

    the second one is invisible.

    this one works
<div class="left_side">

    <script type="text/javascript" src="//form.jotformpro.com/jsform/52155514209955"></script>

    </div>    </tr></table>

    </div><div id="wrapper">

    <div id="container"><div id="div1">

    <table>

    <tr>    <td><div class="top_textResp">

    <h1>Get the Funding you need to Grow Your Business</h1></div></td>

      </tr>

      <tr>

        <th>  <div class="mobi_left_side">form <script type="text/javascript" src="//form.jotform.us/jsform/52627129488162"></script>
this code doesnt (notice urls are the same)
<div class="left_side">

    <script type="text/javascript" src="//form.jotformpro.com/jsform/52155514209955"></script>

    </div>

       </tr>

    </table>

    </div>

    <div id="wrapper">

    <div id="container">

    <div id="div1">

    <table>

    <tr>

        <td><div class="top_textResp">

    <h1>Get the Funding you need to Grow Your Business</h1></div></td>

      </tr>

      <tr>

        <th>  <div class="mobi_left_side">form <script type="text/javascript" src="//form.jotformpro.com/jsform/52155514209955"></script>

    It is recommended you embed the forms using the iframe method, the second is invisible because of cross script conflicts.

    Using iframes eliminates this conflict.
