My form is showing and "unavailable"

    Hello,

I have recently completed a form. However, when I put the link on my website to redirect people to the form, it seems to have trouble opening. It continually says that the site cannot be reached. I have tried to redirect to the form in multiple links and none of them seem to be working. I don't believe its an internet issue either, because the link isn't working from multiple locations.

I asked this question last week and the problem ended up being that I had included asking for credit card information on a form and that caused my account to be disabled. A support staff reached out to me, took away the CC questions and re-opened my account. However, the link still does not work. I have since disabled the CC form as well. Is my account still disabled? Or is this a problem with Jotform? Please let me know.

I have included the website URL. When you look on this page, the form I am trying to access is supposed to be linked to the "Step One" box where it says "Fill in the online application form here"

    Regards.

There are no issues with your account at this time.  I checked your form and it is currently loading without issue.  The only form in your account that is currently disabled is the following:

    http://www.jotform.us/form/71019316957157

That form was manually set to disabled in your account.  It can be enabled from the "My Forms" screen via the "More" tab or from the settings menu while editing the form.
