The Address Section isn't showing up in my form

The address section is not showing up in my form and the name doesn't change when it posts after I edit it

Your address field is conditional and condition was set for the field that was removed from the form. 

To resolve this issue you need to delete or disable that condition. You have two more conditions that are set for fields that are removed from the form. I would suggest you to delete or disable them as well.
