I have checked your account and found that it was automatically suspended by the phishing filter. This filter acts similar to email anti-spam filters which scans and suspends forms without any human intervention.
Upon reviewing your form however, I did not find anything suspicious with any of your fields. As a result, I went ahead and reactivated your account. You should be able to access your forms now.
