My form styles has changed with blue background appearing when previewed

2nd Problem: I added a field to each form used at this URL and when I published it the formatting changed. The form did not have blue coloring in each field previously and for 2 forms ( 3 cans One Time Service and 5 cans One Time Service) the whole monitor screen has the blue coloring.

You have to edit the form. In the Themes tab, remove and re-apply the default theme. This should fix the problem.

 

You are using an old theme with a Blue background. We've updated the theme which is causing the problem in old forms. It needs to be re-applied or replaced with the default theme.
