I checked your form revision history of your form (https://form.jotform.me/71161311853448 ) and I was able to replicate the issue. The Revision History shows blank. I will escalate this issue to our backend team for fixing. Rest assured you will be informed in this thread if there are any updates.

Currently I have other 2 outstanding issues which i have not received any update;

As I discussed this with developer, the earlier changes/revision history can not be seen due to the fact that not being saved theme changes earlier. However, any changes you make on your form will be seen after that.
