    Your account has been suspended due to phishing activity. As you are explicitly collection sensitive credit card information in several forms. Please note that this is a violation to our terms of useYou may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials.

     

    Those fields need to be removed in order for us to re-activate your account. You can use one of our many payment processor integrations, otherwise, we don't allow collecting credit card information in plain text fields.
