Mobile devices get stuck on "please wait" page

Using a laptop or desktop the form functions correctly. Mobile phone users keep getting hung up on a please wait message box. I've tried it on my iPhone 6 and I get this error with both Safari and Chrome browsers. I viewed your forums and our JotForm is not embedded, just a hyperliink on our website.

I've fixed it for you. It's the "Preview Before Submit" widget that's causing the problem. You have hidden some fields in the settings, but the labels are not correct.
